YOUR LIFE, YOUR OWN PACE

Changing your appearance without damaging your health

Read the chapter introduction

This chapter looks at several ways people change their bodies for appearance: severe food restriction, medicines, injections, and surgery. Decisions often arise from comparison with other people, but the consequences fall on the person undergoing the change, and some are irreversible.

What you want to look like is your own decision. The same goal can be pursued through qualified clinicians, prescriptions, and follow-up, or through unverified online products, beauty salons, and self-adjusted doses. This chapter compares the risks of those approaches, focusing on outcomes such as blindness, hospitalization, and death. The cosmetic-procedure entries also discuss criminal liability for unlicensed medical practice in China.

Avoid extreme restriction, fasting, or purging to control weight

In a meta-analysis, people with anorexia nervosa had roughly 5.9 times the expected mortality for their age, with 5.1 deaths per 1,000 person-years; about one in five deaths was suicide. A three-year study across 44 schools found that girls who severely restricted food had 18 times the risk of developing an eating disorder compared with those who did not diet. The source favors physical activity over severe dietary restriction for adolescent weight control. This is not a recommendation to replace restriction with compulsive exercise.

Before injections, thread lifts, or surgery, verify the institution's cosmetic medical authorization and the responsible clinician's qualifications

Invasive cosmetic procedures using needles, surgery, medicines, or medical devices are healthcare. In China they require an appropriately licensed institution with approved cosmetic medical services and registered, qualified clinicians. A beauty salon, informal studio, or hotel room is not an authorized medical setting. Serious unlicensed practice can bring up to three years' imprisonment; serious harm can bring three to ten years; causing death can bring more than ten years.

Recognize the blindness risk of facial fillers, particularly around the nose, between the eyebrows, and forehead

More than half of the published vision-loss cases in one review involved nasal injections, followed by the area between the eyebrows, the forehead, and nasolabial folds. Over 80% involved hyaluronic acid. Only about one-fifth regained full vision, roughly one-sixth regained some, and nearly one-fifth had central nervous system complications. No rescue treatment was consistently effective. These case proportions identify serious hazards, not the probability of blindness from an individual injection.

Avoid unverified pills, coffee, sweets, or preserved fruit promising rapid weight loss

Sibutramine is a drug repeatedly found undeclared in slimming products. China withdrew it in 2010. In a trial of older overweight or obese people with cardiovascular disease or diabetes, it increased nonfatal heart attacks by about 28% and nonfatal strokes by about 36%, while producing only 1.7 kg of additional weight loss after randomization. A product may already have been consumed long before testing reveals an undeclared ingredient.

Avoid nonmedical anabolic steroid use for muscle gain

In a Danish study, men with positive gym doping tests for anabolic androgenic steroids had three times the mortality of matched controls and more than twice the median annual hospital-contact rate. Acne, male breast enlargement, and erectile dysfunction each affected more than one in ten users. In China these substances are prescription-regulated; informal sales without the required controls are unlawful.

If you are considering weight-loss medication, use a legitimate prescribing service

Prescription medicines require a prescription, including online sales. Chinese online sellers must verify a genuine prescription and the buyer's identity. A store that simply ships prescription medicine without assessment or prescription verification is not providing the required safeguards. A clinician should review dose, contraindications, and whether the drug is appropriate.

Use sex hormones with an appropriate prescription and follow-up; do not adjust doses from internet posts

This entry concerns how to use hormones safely, not whether someone should use them. A cohort of transgender women showed higher rates of venous blood clots than matched cisgender controls, with about four additional cases per 1,000 people at two years and 17 at eight years compared with cisgender men. Clinical follow-up helps manage individual risks. Unverified products and unsupervised dosing bypass that care.

If appearance worries become repetitive and disruptive, seek an assessment before arranging surgery

A review estimated body dysmorphic disorder in about 15% of plastic-surgery patients and 13% of dermatology patients; roughly three-quarters of affected participants were women. The condition involves preoccupation with perceived flaws that others barely notice and that disrupt daily life. Repeatedly moving from one perceived flaw to another may not be resolved by changing appearance. An assessment can help guide the decision.
