Increase sleeping altitude gradually, and descend if symptoms worsen instead of pushing on

Grade A evidenceValue: High
In plain language

Sleeping altitude matters, not the highest daytime point. Risk begins around 2,450 meters. Above 3,000 meters, increase sleeping altitude by no more than 500 meters nightly and add a rest day per 1,000 meters gained. Headache with nausea, staggering, mental changes or breathlessness at rest are signals to descend. Descent takes priority over oxygen and medication.

What it takes

Allow one or two additional acclimatization days. Preventive drugs need a prescription and a pre-trip medical discussion. Protect those days even with a tight itinerary.

What you may gain

Sleeping above 2,450 meters exposes unacclimatized travelers to altitude illness. High-altitude cerebral or pulmonary edema—fluid affecting brain or lungs—can be fatal without timely descent.

Context & considerations

Count where you sleep, not daytime peaks. Headache with nausea, impaired coordination, altered consciousness or breathlessness at rest call for descent. Oxygen does not justify continuing upward; descent precedes reliance on oxygen or medicine.

US CDC. Yellow Book: High-Altitude Travel and Altitude Illness. Unacclimatized travelers sleeping at ≥2,450 m (8,000 ft), sometimes lower, are at risk; above 3,000 m (9,800 ft), limit sleeping-altitude gain to 500 m (1,650 ft) nightly and add a day per 1,000 m. The cited acetazolamide prevention regimen is 125 mg twice daily, or 250 mg twice daily above 100 kg body weight. Worsening despite rest/treatment at the same altitude requires descent. https://www.cdc.gov/yellow-book/hcp/environmental-hazards-risks/high-altitude-travel-and-altitude-illness.html; Wilderness Medical Society (2024).
