For serious illness, use insurance, major-illness cover, medical assistance, and out-of-area registration before considering debt

Grade A evidenceValue: High
In plain language

Register before treatment in another province so the eligible insurance portion can be settled directly at discharge, instead of advancing the full bill. Apply for medical assistance if the remaining amount is still beyond you. Employed people may have three to 24 months of statutory medical leave, depending on service history, with protection against specified dismissals during that period. Avoid online high-interest loans and expensive informal borrowing.

What it takes

The applications are free, but the process takes effort while you are ill. First, register before cross-provincial hospitalization through the national out-of-area treatment mini-program accessible from the National Healthcare Security Administration’s WeChat account. Second, use direct settlement at discharge. Third, apply to the township or subdistrict for medical assistance if the remaining bill is unaffordable. Fourth, if employed, arrange statutory medical leave; for a work injury, use work-injury insurance. Fifth, contact the bank to negotiate installments if you cannot meet credit-card payments. Sixth, if funds remain insufficient, consider a Ministry of Civil Affairs-designated personal-assistance fundraising platform. Main documents include the diagnosis, itemized costs, and settlement statement; expect two to four visits.

What you may gain

China describes basic medical insurance, major-illness insurance, and medical assistance as a three-layer protection system. Dibao and special-hardship recipients can receive assistance covering at least 70% of eligible remaining costs. People impoverished by serious illness can apply for additional assistance. Registering for cross-provincial hospitalization enables direct settlement rather than paying everything upfront and returning home to claim reimbursement. Statutory medical leave for illness or non-work injury lasts three to 24 months depending on total service and service with the current employer. Dismissal protections apply during that period, and sick-leave pay must not fall below 80% of the local minimum wage. For a recognized work injury, the period away from work with retained pay normally lasts no more than 12 months, with original wages and benefits paid monthly by the employer. A credit-card holder whose debt exceeds repayment capacity but who is willing to repay may negotiate an individualized installment agreement lasting up to five years; after agreement, the bank must stop collection activity under the rule’s conditions. Online platforms facilitating personal hardship appeals must be designated by the State Council’s civil-affairs department and verify the appeal. Donations may be transferred only to the applicant or an account the applicant provides.

Context & considerations

Medical-assistance percentages and annual caps are set where you are insured. Medical-leave entitlement has six bands based on total working years and time with the employer. Once it expires, lawful termination may become possible; establish your band early. Emergency treatment also appears among situations requiring out-of-area registration, but the cited page did not establish whether emergency registration is automatic or can be completed afterward; ask your insurance locality. The court-protected interest ceiling for private lending is four times the one-year LPR, discussed in avoiding predatory borrowing. Interest above that ceiling is unprotected, but the principal remains repayable, and someone in hospital may have little energy for litigation. Acute life-threatening illness or injury is different: call China’s emergency number 120 even if you have no money. Hospitals must not refuse, divert, or delay emergency treatment, and the emergency medical-assistance fund can cover qualifying emergency costs; see emergency care without upfront payment. The 1995 labor-ministry document was checked through the provincial repost because the ministry’s original page was unavailable.
